Driver Assist & Collision Avoidance complaints by model year
My 2025 Hyundai Tucson (VIN [XXX]) has been out of service since [XXX] (over 60 days) at CMA’s Colonial Hyundai in South Chesterfield, Virginia, waiting for a front camera related to safety Recall 302 (NHTSA Campaign 26V316 – Front View Camera FCA Update). The dealership previously replaced the front camera, but the issue was not resolved. They then ordered a new camera that remains on backorder with no firm ETA. I have been provided a rental during this time. Hyundai assigned Case #[XXX]. I have provided formal written notice under Virginia lemon law demanding a buyback because the vehicle has been out of service far beyond the 30-day threshold. The safety-related defect remains unrepaired. Components involved: Forward Collision Avoidance / Front Camera / Automatic Emergency Braking INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
NHTSA complaint #11758709 — an unverified report by a vehicle owner.
When the vehicle in front of me stopped quickly, I braked quickly but did not hit the car. At that time, all the lights on my dashboard went off and were still on when I took the car to dealer for a diagnosis.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
I always have the Forward Attention Warning turned off in Vehicle Settings. This past week, 8/13/2026, while on the freeway I was doing the speed limit which was 70 mph. I had the Cruise Control turned on as well as the Lane Assist. While briefly changing the map setting on the display and not looking forward I got the warning “Eyes on the road, Hands on the wheel”. I got that warning twice within a minute. I was in the middle lane of a 3 lane freeway with fairly heavy traffic. All of a sudden there was loud beeping and the flashing red message “Take Control Now, Vehicle Stopping” The cruise control disengaged and the brakes were applied. My speed dropped from 70 down to 55 before I was able to regain control. Two minutes lather the exact same thing happened for a second time. Needless to say the traffic behind me also had to brake to avoid hitting my car! I have done some further testing and discovered while the Cruise Control is turned on the infrared camera behind the steering wheel is always active, regardless of what I have selected in Settings. If the Cruise is not turned on then the infrared camera will not function unless it is has been turned on in Vehicle Settings. I was very lucky that I didn’t cause an accident. Sooner or later someone isn’t going to be as lucky. The Forward Attention Warning System needs to be removed as a “Safety” feature. It is an accident waiting to happen! Another question is what will the effect be on my eyes after hours of exposure to the infrared camera beams? Has that been evaluated from a health standpoint? I don’t see any positive benefits of having the infrared beams constantly beaming into my eyes for hours and hours on a long trip.

My 2025 Hyundai Tucson required the front brake rotors to be resurfaced at approximately 12,200 miles. The Hyundai dealership diagnosed the rotors as overheated and warped, despite the brake pads being in good condition. The repair occurred before Hyundai issued Recall 302 (NHTSA Recall 26V-316). Hyundai Recall 302 identifies a defect in the Forward Collision-Avoidance (FCA) system’s front-camera software that can cause the FCA system to engage earlier than expected and potentially result in sudden braking. Hyundai’s FCA system is capable of automatically applying the brakes. I am concerned that premature or unintended brake application associated with this recalled software condition may have contributed to excessive brake heat and premature overheating/warping of my front rotors. I am NOT claiming that this causal relationship has been established. Rather, I am reporting a potential safety defect and asking NHTSA to evaluate whether there may be a relationship. The dealership confirmed that the rotors were overheated/warped but characterized the condition as wear and tear. Hyundai Motor America subsequently denied my reimbursement request on the same basis. I have repeatedly asked Hyundai to evaluate whether Recall 302 could have contributed to the brake condition, but Hyundai has not acknowledged or technically addressed this potential relationship. The original rotors are no longer available for inspection because they were resurfaced during the repair. The dealership performed and documented the diagnosis and repair. I am submitting this complaint because I believe the combination of premature rotor overheating/warping at only 12,200 miles and a subsequently identified FCA defect involving unexpected braking warrants investigation.

While backing out of driveway, with parked vehicle along side and one moving vehicle in the road, I was applying the break and the Emergency Stop feature activated. My speed was 2-3 MPH and slowing.
NHTSA complaint #11755478 — an unverified report by a vehicle owner.
Emergency Brake applied while driving. Had been driving with vehicle in front, on each side and one at rear with issues and all of sudden the emergency brake was applied causing the vehicle in back to get very close to my car. This is has happened multiple times and reported to dealership multiple times. This was the second occurence on this day.

The contact owns a 2025 Hyundai Tucson. The contact stated that while driving at various speeds, the Forward Collision Avoidance Assist (FCA) warning light was intermittently illuminated. The dealer was notified of the failure and attributed the issue to inclement weather. The contact stated that while stopped, the vehicle was rear-ended because the brake lights failed to illuminate. The contact sustained lower back and shoulder injuries, was transported by ambulance to the hospital, and required ongoing injection treatments. A Police report was filed. The vehicle was towed to a dealer and repaired for rear-end and passenger-side damages; however, the cause of the brake lights' failure was not determined. Additionally, the contact stated that while attempting to accelerate from a complete stop at a traffic light, the vehicle stalled and was struck on the front driver's side by another vehicle. The crash caused the vehicle to spin, and the front end detached. The air bags failed to deploy, and a system failure message was displayed. The contact sought medical attention the following day. The contact sustained a dislodged left thumb bone which required surgery, a cast, and injections for left arm pain, as well as shoulder, neck, hip, and aggravated lower back pain. A Police report was filed. The second failure had occurred at [XXX]. The vehicle was towed to the auto body shop at the dealer and repaired; however, neither the dealer nor the insurance adjuster determined the cause of the vehicle stalling. The contact associated the failure with an unknown recall notification. The manufacturer was not notified of the failures. The failure mileage was unknown. Forward collision Warning and Automatic Emergency Braking systems failed on 5/13/2026. Driving at a low rate of speed, approximately 15mph in a roundabout traffic circle with no other vehicles in front of the vehicle, sunny dry weather, 740am. Proceeded into traffic circle and FCA engaged and hard braking occurred for instantaneous stop and my vehicle was hit in the rear by a school bus. Air bags did not deploy. No recall on the vehicle at time of accident. Recall issued 6 days after accident for FCA. After contacting insurance company, Hyundai was contacted and made aware of the incident and safety concerns. The vehicle was towed to the dealership that evening and the next day I was advised that they could not check out the vehicle prior to having the collision repaired. The vehicle was then towed to a collision repair shop and released from there and towed back to the dealership on 6/19/2026. The vehicle had two safety recalls at that time and according to the service department both have been addressed. The vehicle is presently sitting the dealership (8/4/2026) waiting for Hyundai to investigate further. There were a few incidents prior to the collision that occurred randomly with the warning going off when no other vehicles or structures were close by. I took possession of this vehicle on 11/22/2025 and it had 2,431 odometer miles when this accident happened.

The contact owns a 2026 Hyundai Tucson. The contact was notified the following day after purchasing the vehicle that there was an open recall on the VIN. The VIN was included in NHTSA Campaign Number: 26V316000 (Forward Collision Avoidance). The parts were available for the recall repair; however, the dealer where the vehicle was purchased was 97 miles away. The contact was advised to take the vehicle to the local dealer for the recall repair. The repair was in the process of being completed on 08/03/2026. The contact researched and became aware that the “Do Not Sell” restriction was in place and was dated 5/19/2026. The manufacturer was not made aware of the issue.

Unintentional and unexpected activation of emergency collision or adaptive cruise control, causing the vehicle to stop and become immobile in the lane of traffic. I was driving down the interstate with the adaptive cruse control on and the lane keeping assistance on. These both were on for about 20 minutes as I was driving down the highway. There were no vehicles in front of or behind me for a quarter mile or so. I as all alone with nothing around me. All of a sudden it seemed like the collision avoidance system activated. The car came to a complete stop in the right lane of the highway. I tried to proceed and could see the emergency brake was on, the engine was running the car was in drive but it would not move. Flipped on the hazard flashers and traffic caught up to me while sitting in the right lane of the highway waiting to get rear ended. Vehicles were barely getting by at 70 MPH. The car would I pressed the accelerator and the car move d forward enough to get off the highway, this took a minute or two. Once off he highway, I could see the emergency brake would not release. I turned the engine off went out to inspect all the forward sensors. There was no visible issue. I restarted the car after a minute or so and it proceeded normally. That was Saturday, this is Sunday, I am taking the vehicle to Hundai tomorrow for inspection.

On July 24, 2026, while driving on a marked city street with no vehicle, pedestrian, or obstruction ahead, the Forward Collision-Avoidance system on our 2022 Hyundai Tucson Limited AWD applied emergency braking on its own, three to four separate times, each lasting one to two seconds. Traffic behind us nearly struck our vehicle. My family was in the car. This is the latest event in a progressive electrical fault history. In June 2025, under warranty at 48,037 miles, a Hyundai dealer documented recurring multi-system warning cascades (forward safety, lane keeping, high beam assist, speed limit warning) and a failed auto start/stop system. The dealer's listed resolution was "battery voltage / replace battery"; their invoice records that no repair was performed. We replaced the battery at our own expense. The faults continued. In June 2026, the start/stop system shut the engine off at a red light and the vehicle would not restart, leaving us disabled in a live traffic lane. Power was only restored by cycling the hood release. Since then, TCM and ECU software updates and a battery harness replacement (our expense, approx. $1,200) have been performed. The faults persisted after these repairs and have now escalated to uncommanded emergency braking. Date- and time-stamped video and photographs exist for multiple events. Two Hyundai dealerships have not identified a root cause. Similar phantom braking reports exist from other owners of this model year. We no longer consider the vehicle safe to drive.

This is not the first situation NOR the first time it is being "fixed". I took my car in early March for an oil change and informed the representative about the same thing. The time that made me say something is when I was switching lanes and the car must have sensed me being close causing me to stop completely and the car behind me having to hard break and swerve. Rick Case Hyundai in Duluth, Georgia had my car for a MONTH, just for me to experience the same thing over 3 times since then. I received my car in April, and the recall for this wasn't put out until May. I am constantly scared for my life and the passengers I may have lives, as well as the other drivers lives, considering I got this car due to being in a rear end collision. When I took it in April, they CONSTANTLY, drove my car to diagnose it but kept telling me they didn't see the problem, and it wasn't coming up for them on their internal tracking system for their cars(webDCS). Once they finally talked to the CORPORATE field technician, they advised they replace the whole front bumper, including sensors, for the solution. But like stated before, since then I have experienced near crash stops AT LEAST 3 times since then, two of them being in the same month (July 16 and July 28). If this is the same solution, I don't see it being beneficial and I would like to be bought out at this point. Especially if I have to be without my car for a long period of time again.

All the warning lights in the day lit up. Brake, LDWS, AEB, outside air temperature won’t read and the a/c blows hot. $486 to diagnose at dealer and they state that it’s an electrical issue due to a bad vacuum pump. I can’t get an answer as to whether the vacuum pump safety campaign was completed or not. Dealer wants $1500 to replace.
