Owners have filed 1,304 complaints
about the 2021 Ford F-150 with NHTSA between 2021 and 2026.
The largest single group is the transmission & drivetrain.
Across this model's run it ranks 29 of 33 worst.
The letter says how bad. This says what kind — whether this year drew
an unusual number of complaints, unusually dangerous
ones, or failures that arrived unusually early.
The dashed ring is 50 — the national norm. Further out is worse.
Complaint volume — 46
Against the median year of the Ford F-150. 50 means typical for this
model; 100 means roughly double.
Severity — 13
2.6% of these complaints
involved a crash, fire, injury or death, against a national rate of
9.7%.
Early failure — 75
44% of failures with a mileage on record happened under
36,000 miles, against a national rate of
29%.
NHTSA files every complaint against a named sub-assembly, not just a
system. This is the difference between knowing it was "the engine" and
knowing which part of it — the list to hand a mechanic.

NHTSA investigations into the 2021 Ford F-150
An investigation is the step before a recall: the agency has seen enough
complaints to look into a pattern itself. An open one means it thinks
something may be wrong and nothing has been fixed yet.

Loss of Motive Power
On July 22, 2022, the Office of Defects Investigation (ODI) granted a Defect Petition (DP22001) and opened a Preliminary Evaluation (PE22007) to investigate allegations of loss of motive power in model year (MY) 2021 Ford Bronco vehicles equipped with 2.7L EcoBoost engines. Complainants were reporting sudden engine failure while driving resulting in a loss of motive power, often without the ability to restart. During PE22007, the cause of engine failure and loss of motive power in the subject vehicles was determined to be intake valve fracture, and the full scope of vehicles affected by the su...
Recalls on the 2021 Ford F-150
A complaint is one owner's allegation. A recall is the point at which
the manufacturer or a regulator agreed the defect is real and dangerous
enough to fix for free — however old the car is.

BACK OVER PREVENTION:DISPLAY FUNCTION
Ford Motor Company (Ford) is recalling certain 2021-2023 F-150 and 2023-2026 F-250 SD vehicles equipped with 8-inch SYNC infotainment screens. A software error may cause the rearview camera image to be obstructed by the infotainment home screen menu when the vehicle is placed in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Risk: An obstructed rearview camera image can reduce the driver's rear visibility, increasing the risk of a crash.
26V402000
POWER TRAIN:AUTOMATIC TRANSMISSION
Ford Motor Company (Ford) is recalling certain 2018-2021 Navigator, Expedition, 2020-2021 Explorer, Lincoln Aviator, and 2021 F-150 vehicles. The transmission park pawl may engage while the vehicle is in motion, resulting in park system damage.
Risk: Park system damage may result in a vehicle rollaway, increasing the risk of a crash.
26V104000
ELECTRICAL SYSTEM: INTEGRATED TRAILER BRAKE CONTROL
Ford Motor Company (Ford) is recalling certain 2021-2026 F-150, 2022-2026 F-250 SD, F-350 SD, F-450 SD, F-550 SD, Maverick, 2024-2026 Ranger, 2023-2026 F-600 SD, 2022-2027 Lincoln Navigator, Expedition, and 2026 E-Transit vehicles. When towing a trailer, the integrated trailer module (ITRM) may lose communication with the vehicle, possibly causing a loss of brake and turn signal lights, or a loss of brake function. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 108, " Lamps, Reflective Devices, and Associated Equipment."
Risk: Trailer lights or brakes that do not function can reduce the driver’s ability to control the trailer and decrease the trailers visibility to other road users, increasing the risk of a crash.
25V729000
VISIBILITY:SUN/MOON ROOF ASSEMBLY
Ford Motor Company (Ford) is recalling certain 2021-2022 Expedition, Lincoln Navigator, 2021-2023 F-150, 2022 F-250 SD, F-350 SD, and F-450 SD vehicles. The moonroof wind deflector clips may be inadequately attached to the moonroof frame, which can result in the wind deflector detaching from the vehicle.
Risk: A detached wind deflector can become a road hazard and increase the risk of a crash.
25V455000
FUEL SYSTEM, GASOLINE:DELIVERY:FUEL PUMP
Ford Motor Company (Ford) is recalling certain 2021-2023 Bronco, Explorer, Lincoln Aviator, F-250 SD, F-350 SD, F-450 SD, F-550 SD, 2021-2022 Lincoln Navigator, Mustang, F-150, and 2022 Expedition vehicles. The low-pressure fuel pump may fail, which can result in an engine stall while driving.
Risk: An engine stall increases the risk of a crash.
25V348000
EXTERIOR LIGHTING:BACK UP LIGHTS
Ford Motor Company (Ford) is recalling certain 2021 F-150 vehicles equipped with the Onboard Scales feature that were repaired incorrectly under recall 24V443. An over-the-air software update to the body control module (BCM) may have disabled both reverse lights.
Risk: Inoperative reverse lights may fail to alert others that the vehicle is reversing, increasing the risk of a crash.
25V315000
BACK OVER PREVENTION:SOFTWARE
Ford Motor Company (Ford) is recalling certain 2021-2024 Bronco, F-150, 2021-2024 Edge, 2022-2025 Escape, F-250, F-350, F-450, F-550, F-600, 2022-2024 Expedition, 2022-2025 Transit, 2021-2023 Mach-E, 2024 Ranger, Mustang, 2021-2023 Lincoln Nautilus, 2022-2024 Navigator, and 2023-2024 Corsair vehicles. A software error may cause the rearview camera image to delay, freeze, or not display when the vehicle is in reverse.
Risk: A frozen or missing rearview camera image can reduce the driver's view behind the vehicle, increasing the risk of a crash.
25V169000
POWER TRAIN:AUTOMATIC TRANSMISSION:CONTROL MODULE (TCM/PCM/TECM)
Ford Motor Company (Ford) is recalling certain 2021 F-150 vehicles previously repaired incorrectly under recall number 22V-188. The Diagnostic Trouble Code (DTC) for low transmission fluid pressure may falsely activate and cause the transmission to unexpectedly shift into neutral.
Risk: An unexpected shift into neutral will cause a loss of drive power and increase the risk of a crash.

Recall 20263697,362 units in Canada
Visual System
Issue: Under certain conditions, a software problem could cause part of the rearview camera image to be blocked. Canadian regulations require the full rearview image to display while the transmission is in reverse gear. Note: This recall only affects vehicles equipped with an 8-inch display and 360-degree cameras. Safety Risk: A rearview camera image that is not fully displayed could reduce a driver's ability to see behind the vehicle while backing up. This could increase the risk of a crash. Corrective Actions: Ford will notify owners by mail and send a wireless over-the-air software update to eligible vehicles to update the APIM software. Alternatively, you may schedule an appointment to have the update performed at a Ford dealership.
Recall 2026342105 units in Canada
Lights And Instruments
Issue: This recall is for certain integrated trailer modules that were installed before delivery or purchased from a dealer as an aftermarket accessory. On certain vehicles, a software problem with the integrated trailer module (ITRM) may cause the trailer lights not to work when towing. This problem could also cause the trailer brakes not to work on certain vehicles. Note: This problem would cause a "Trailer brake module fault" message to be displayed and turn signal telltale to flash rapidly. Safety Risk: Trailer lights that don't work could make the vehicle less visible and cause following drivers not to recognize that the vehicle is braking or intends to turn or change lanes. These issues could increase the risk of a crash. Additionally, a loss of trailer brakes could increase the risk of a crash. Corrective Actions: Ford will notify owners by mail and advise you to take your vehicle to a dealership to update the ITRM software.
Recall 202631133,012 units in Canada
Powertrain
Issue: On certain vehicles, a problem in the transmission could cause the parking pawl to become damaged. As a result, when the transmission is shifted to Park, the vehicle could move if the parking brake is not applied. Safety Risk: A parked vehicle that rolls away could create the risk of an injury or a crash. Corrective Actions: To reduce the safety risk, Ford recommends that you should always use the parking brake when you park until the recall repairs are completed. Ford will notify owners by mail and advise you to take your vehicle to a dealership to update the powertrain control module (PCM) software. The dealer will also inspect and, if necessary, replace any damaged transmission components.

Powertrain
Issue: On certain trucks, the recall repairs completed by a Ford dealer during recall 22S15 (Transport Canada recall no. 2022-144) may not have been done correctly. As a result, a second repair is required. On certain trucks, the powertrain control module (PCM) may have been programmed incorrectly. As a result, a false diagnostic trouble code (DTC) could cause the transmission to shift into neutral while driving, causing a sudden loss of power to the wheels. If the vehicle speed drops below 3 km/h after this happens, the transmission can shift to park automatically. Note: This recall only affects trucks equipped with a 2.7 L, 3.5 L, or 5.0 L engine and without auto start-stop technology. Safety Risk: A loss of power to the wheels could increase the risk of a crash. Corrective Actions: Ford will notify owners by mail and advise you to take your truck to a dealership to update the powertrain control module (PCM) software.

UNKNOWN OR OTHER
On Sunday, [XXX], I was driving on a state highway in my 21 Ford F150 FX4 with my family. 67mph, 73 degrees sunny outside. The front portion of my moonroof/sunroof exploded, shooting glass shards internally all over my truck/passengers and externally into the bed of my truck, and into the boat I was towing. Multiple people had glass shards embedded in their skin. There were no other vehicles nearby, and there was no prior damage to the sunroof/moonroof of the vehicle. The truck gave no warning or signs of malfunction prior to the event. The sunroof was closed, and all windows in the vehicle were closed. On Monday morning ([XXX]), I took my truck to the local Ford service center, and they inspected it, shocked at the results. They filed a claim that Ford denied because the windows are not covered by my extended warranty. They wanted to charge me $ 4,000 for the repair/replacement of the glass and track. I have been on the phone with Ford Protection and Ford Customer Service (Aug 6th, 2025), trying to get something resolved, and Ford continues to deny responsibility. My vehicle was left rendered unable to steer during use. It was taken to a local shop All Japanse Auto, who found the damage was consistent with Ford Saftey Recall 21S28. Upon speaking to AutoFair Ford of Haverhill service they informed me Ford did not include my truck in the recall despite it matching year and model and they would not be addressing the issue for me. VEHICLE SPEED CONTROL
Due to a dangerous design flaw in the 2021 Ford F150, I was put in a potentially dangerous situation by completely losing all power in the vehicle on a backroad. Design flaw is the DCN module, which floods into the vehicles electrical system/body of vehicle through the cowel drain that can supposedly be blocked by a singular piece of pine straw. Due to the design flaw, the vehicle completely lost all power and all electronics failed while driving down the road at 45 mph, putting both me and my father (riding passenger seat) in immediate danger. Ford knows and is aware of the design flaw and has refused to issue a recall, not only affecting my safety, but now I am going back and forth with ford over the fix to the tune of $2800 that they refuse to fix (vehicle is a gold certified cpo, has extended warranty). When asked how to prevent in the future, ford was and is not aware of how to prevent in the future, and I can not get to the cowel drain clog without professional equipment and ford performing. There is no way to tell the cowel drain is clogged or not, and there is not anything in the ford owners manual related to maintenance of the cowel drain. Again, due to the inappropriately designed drain, this caused water leakage into my truck, posing an immediate threat to my life and others around me. I have all of the videos and pictures needed to show the failure of the system to provide to move this forward to get resolved. I had complete loss of power and electronics on the road. I tried to attach video but it would not let me. I can attach via email when requested.

FUEL/PROPULSION SYSTEM
On June 20, 2026, my 2021 Ford F-150 5.0L V8 suddenly lost engine power while I was driving on the freeway. The vehicle became disabled and had to be towed. The vehicle was towed to an authorized Ford dealership on June 23, 2026, where it remains. The dealership inspected the vehicle and confirmed that it is affected by Ford Recall 25S75 / NHTSA Recall 25V455 and advised that there is currently no available repair or remedy for the fuel pump issue. Ford Customer Service has also opened a case regarding the vehicle and provided a loaner vehicle, but no repair or estimated date for a remedy has been provided. The sudden loss of engine power while traveling on the freeway created a significant safety hazard. The vehicle could no longer maintain normal speed or acceleration, creating a risk of being struck by other vehicles traveling at highway speeds and a risk to surrounding motorists. Ford's recall identifies this fuel pump issue as capable of causing an engine stall while driving. The vehicle remains at the Ford dealership and is available for inspection upon request. To my knowledge, it has not been inspected by police or an insurance representative. There were no known warning symptoms prior to the failure; the vehicle was operating normally until it suddenly lost engine power on June 20, 2026. The failure is particularly concerning because the recalled component appears to have failed in the exact manner described in the recall, leaving the vehicle disabled while driving. Ford currently has no available remedy, and the vehicle remains unrepaired at the dealership.

I've noticed that the vehicle struggle when acceleration, specially at full stop (stoplight and intersection). Feels like the engine is going to die out if I step on gas pedal.

The contact's son owns a 2021 Ford F-150. The contact stated that after her son shifted from park(P) to drive(D), the vehicle failed to respond as needed. Several unknown warning lights were illuminated. The contact's son was able to drive to the residence nearby after several attempts. While at the residence, the failure recurred after shifting to drive(D), and the vehicle made an abnormal grinding sound. The vehicle was towed to an independent mechanic, and it was determined that the passenger’s side rear axle was fractured. The failure was associated with NHTSA Campaign Number: 23V896000 (Power Train). The vehicle was towed to a dealer, where the diagnosis was confirmed; however, parts were unavailable for the recall repair. The vehicle remained unrepaired, and no rental vehicle was provided. The manufacturer was notified of the failure, and a case was filed. The failure mileage was approximately 100,650.

My F150 10R80 transmission is throwing a P076F code. And the shifting has become very harsh and eratic all across the 10 gears. Hesitations, high revs, hard shifts, etc. This is a known issue by Ford and they have issued TSBs on the issue. They acknowledge that it is a design flaw within the transmission and have issued fixes. But if you are outside of the 60,000mi warranty, the burden of the costs falls on me, the owner. $9400 to replace the transmission with a new Ford transmission here in Colorado. This is a known problem in the design but Ford is not stepping up to help. They let the burden fall on the owner.

FUEL SYSTEM, GASOLINE
The contact's son owns a 2021 Ford F-150. The contact stated that while the vehicle was parked, the owner attempted to start the vehicle; however, the vehicle failed to start. The owner replaced the battery, and the vehicle was towed to the dealer and was diagnosed with a fuel pump failure. The contact was informed that the fuel pump needed to be replaced. The contact was informed that parts were unavailable for the repair. Additionally, the owner was informed that the repair could be paid for out-of-pocket and a request for reimbursement could be made when parts became available. The owner declined to pay out of pocket for the repair. The vehicle was not repaired. The dealer requested that the vehicle be removed from the dealer lot. The vehicle was towed to an alternative dealer for storage because the vehicle was inoperable. The owner then received notification of NHTSA Campaign Number: 25V455000 (Fuel System, Gasoline); however, the part to do the recall repair was unavailable. The manufacturer was made aware of the failure, and two cases were filed. The failure mileage was approximately 66,000.

This recall has been going on for over a year. I don't know how long these usually take, but this recall - Manufacturer Recall Number25S75 NHTSA Recall Number25V455 Recall StatusRecall Incomplete, remedy not yet available My truck runs rough at times which I believe is a direct result from the fuel pump on this recall, and it doesn't give me much confidence on my daily driver truck, especially going on long trips. I don't know if this message carries any weight in the timeline on this recall, but I would really like this fixed soon so I don't have a stall incident with my family in the vehicle.

On-going issue with 1080R transmission acting weird. Hard downshifts when truck is cold, even slightly warmed up. Lazy shifts, like transmission gets lost shifting from 2nd to 3rd. It pauses the shift and takes about 5 or 6 seconds to find 3rd gear. Seems to be in line with tons of known Ford 1080r cdf drum issues based on info I read online. Dealer has noted and replicated some of the issues. They've done a relearning procedure which temporarily seems to help it but then goes back to issues within a few hundred miles.
