From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. In December 2019, the Office of Defects Investigation (ODI) opened this investigation based on 235 Vehicle Owner Questionnaires (VOQs) alleging doors failed to latch when closed, and/or inadvertently opened while driving, on model year (MY) 2012-2014 Ford Fiesta, and 2013-2014 Fusion and Lincoln MKZ. Ford Motor Company (Ford) is recalling certain 2014-2015 Fusion, Lincoln MKZ, and 2015 Mustang vehicles originally sold or ever registered, in Texas, Louisiana, Mississippi, Alabama, Florida, Georgia, South Carolina, North Carolina, Virginia and Hawaii. Exposure to certain environmental conditions, such as high temperatures, high humidity, and salt air can cause the brake pedal bumper to corrode and separate from the brake pedal.

Risk: A separated brake pedal bumper can cause the brake lights to illuminate continuously, causing confusion to other drivers and increasing the risk of a crash. On automatic transmission vehicles, a missing brake pedal bumper can allow the driver to shift the vehicle out of the "PARK" position without depressing the brake, allowing the vehicle to rollaway, increasing the risk of injury.

Ford Motor Company (Ford) is recalling certain 2011-2014 Fiesta, 2013-2014 Fusion, 2015 Mustang, 2013-2015 Escape and C-Max, 2012-2015 Focus, 2014-2016 Transit Connect, 2013-2014 Lincoln MKZ, and 2015 MKC vehicles. These vehicles were previously recalled and repaired under recalls 15V-246, 16V-643 or 17V-210, however the repair may not have been completed properly. A component within the door latches may break making the doors difficult to latch and/or leading the driver or a passenger to believe a door is securely closed when, in fact, it is not.

Risk: A door that is not securely latched could open while the vehicle is in motion, increasing the risk of injury to a vehicle occupant.

Ford Motor Company (Ford) is recalling certain model year 2015-2017 Mustang vehicles manufactured February 24, 2015, to August 30, 2016. A hose may separate from the engine oil cooler tube assembly causing an oil leak.

Risk: A sudden loss of engine oil may cause engine failure, increasing the risk of a crash. Additionally, an oil leak, in the presence of an ignition source, increases the risk of a fire.

SERVICE BRAKES, HYDRAULIC:FOUNDATION COMPONENTS:DISC:CALIPER

Roush Performance Products, Inc. (Roush) is recalling certain model year 2015 Ford Mustang GT Roush Stage 3 vehicles manufactured April 1, 2015, to October 31, 2015, and equipped with Roush Extreme Duty Half Shafts. The affected vehicles have a brake caliper guide pin that may not have been sufficiently tightened during installation of the half shafts. As a result, the caliper guide pin may loosen while driving, reducing the vehicle's rear braking ability.

Risk: A partial loss of the rear braking system can increase the risk of a crash.

Issue: On certain vehicles, the driver and passenger-front seat belt pretensioner cable could corrode over time. Issue: Ford Motor Company of Canada is conducting a parts recovery campaign targeting a random sample of 2015-2017 Ford Mustang vehicles in the Montreal area built between February 11, 2014 and October 2, 2017. The driver and front passenger seat belt assemblies are being recovered to aid in Ford's analysis in support of Transport Canada defect investigation file no. 3280-38-20. Note: This action is not being conducted under the requirements of the Motor Vehicle Safety Act. Safety Risk: No safety defect has been identified with these seat belts. Corrective Actions: Ford will notify select owners by mail and advise them to take their vehicle to a dealership to replace the driver and front passenger seat belts at no charge.

On certain vehicles, the pawl spring tab in the side door latch could break. This condition typically will prevent the door from latching. In certain situations where the door is able to be closed, the door may be partially latched and could unlatch while driving. This could increase the risk of injury to a vehicle occupant seated next to the door, or result in damage to property. Correction: If a door will not latch, dealers will replace all four door latches with a more robust part. Note: A door that is partially latched can cause the courtesy light to remain on, set a door ajar message in the instrument cluster and sound a door ajar chime to alert the driver that there is a door latch problem.

Fuel System

On certain vehicles equipped with a 2.3L engine, prolonged exposure to elevated underbody temperatures could cause the fuel tank, fuel vapor lines and/or parking brake cable seals to degrade. This could result in a fuel leak which, in the presence of an ignition source, could result in a fire causing injury and/or property damage. This could also affect parking brake function, which could result in a rollaway event if the vehicle was left unattended and the parking brake was not applied, which could increase the risk of a crash causing injury and/or property damage. Correction: Dealers will replace the fuel tank shield, apply thermal patches to the fuel tank, parking brake cable and fuel vapour lines.

On certain vehicles equipped with a 2.3 Litre engine, the fuel pressure sensor may not have been properly seated to the fitting on the fuel jumper line. A partially seated fuel pressure sensor could result in a fuel odour and/or a pressurized fuel leak. A fuel leak, in the proximity of a hot surface that might be a potential ignition source, may result in a fire causing injury and/or property damage. Correction: Dealers will replace the fuel jumper line.

I exited the highway to get out of danger, it then had a wrench light ( transmission) pop up and when I parked the car on the access road it started smoking a lot. I turned the vehicle off immediately and when I seen smoke coming from under the hood and into the cab of the car. I lifted the hood after seeing fire in the passenger wheel well. And then in the engine block towards the windshield in the middle of the engine. I am a fire fighter and it appeared to me that the fire was being fed by some kind of substance. Fort Worth Fire department showed up and put the fire out and took a report.

Irmo, SC Fire

ENGINE

“Charge system service now” came on the dash, battery lost power after about 10 seconds, screen went black, dash went black, car locked up, opened the hood and the engine was on fire, called 911 who came quickly and put the fire out without the entire car burning, insurance later total losses the car. I don’t know if it was the battery or the engine responsible for the failure, because the flames were on the engine and not the battery, but the incident was triggered by a battery warning. I was on the interstate driving 60mph when the car shut off and everything including my headlights and even hazard lights went black. Not only could I have been struck by a car on the interstate, I could have died in a car fire. Because the car locked up, I couldn’t open the trunk or use any electronics. The door could have just as easily locked and trapped us inside a burning car. The car is on a salvage yard, no attempt to replicate has been made. Progressive insurance, and Columbia fire inspected it and found that it was undetermined. “Charge service system now” was the only warning before the infotainment screen shut down to reduce battery usage. Every light on the dash flashed right before everything on the dash went black and power was lost to the entire vehicle.

Janesville, WI Crash

BACK OVER PREVENTION

The contact owns a 2015 Ford Mustang. The contact stated that the back over prevention camera intermittently failed to operate as needed while the vehicle was shifted into reverse(R). The contact stated that the camera image either displayed a blank image or the camera displayed an inaccurate distance between objects. As a result, the contact stated that while reversing into the driveway at approximately 5 MPH, the rearview camera displayed an inaccurate image and the distance between the vehicle and another vehicle parked in the driveway. As a result of the discrepancy, the contact struck the parked vehicle, which damaged the rear quarter panel of his vehicle but there was no damage to the other vehicle. Due to the low impact of the crash, the contact was not injured and an air bag did not deploy. There was no Police report filed. The contact then received notification of NHTSA Campaign Number: 25V572000 (Back Over Prevention); however, the part to do the recall repair was not available. The local dealer was made aware of the issue and confirmed that parts were not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was notified of the failure but offered no assistance. The failure mileage was unknown.

i was driving on a highway and experienced a total failure of my electrical system which completely shut down with in 15 seconds i lost control of the car as i could not even move the wheel, subsequently T-boned by another car iwhich could not stop fast enough suffering total loss of my car of note prior to the accident (Sunday AM) on friday I had a battery replaced @ Firestone, see reciept attached that was the last repair for the care

POWER TRAIN

Clutch assembly failed, the metal bracket weld cracked/failed resulting in the clutch pedal being inoperable. It's not available for inspection since I had it replaced and the defective assembly disposed of. My safety was at risk since I had extreme difficulty shifting gears on the road. The issue was confirmed at an independent service center - AAMCO of Huntsville, AL. No other entity inspected the part. The vehicle gave me no warning or indication of a problem other than the difficulty with starting and operating it.

I am filing a formal safety complaint regarding a catastrophic Electronic Power Assisted Steering (EPAS) failure and severe electrical short circuit on my 2015 Ford Mustang. This vehicle is exhibiting hard fault code B137F:09 (Steering Wheel Left Switch Pack: Component Failure / Short to Ground or Open). This electronic failure creates an immediate, severe safety hazard that goes entirely outside the scope of normal vehicle wear and tear. While driving, the vehicle's steering wheel pulls and jerks violently on its own, actively fighting my manual steering inputs. Furthermore, this internal electronic short creates a massive parasitic electrical draw that prevents the vehicle's computer modules from entering sleep mode when turned off. This has resulted in the primary vehicle battery draining completely flat and causing the vehicle to completely shut down, stall, or lose total electrical power altogether. A vehicle that forcefully steers itself away from the intended path of travel and suffers total electrical shutdown is a clear, life-threatening safety risk to myself and others on the road. Ford Motor Company is fully aware of this dangerous defect. They previously issued Safety Recall 15S18 for a very specific, limited batch of early-production 2015 Mustangs experiencing identical EPAS failures, replacing their steering gears for free. For the remaining 2015 Mustang owners experiencing the exact same hazardous failure, Ford quietly shifted the issue over to a Technical Service Bulletin (TSB / SSM 51312). By limiting the scope of Recall 15S18 purely due to the financial cost of a universal recall, Ford has left an entire population of 2015 Mustang owners in direct harm's way. Some owners received a free lifetime safety fix, while the rest of us are being forced to pay thousands of dollars out-of-pocket to correct an identical factory electronic defect. I request that the NHTSA immediately open an investigation into Ford's handling of the 2015 Mustang EPAS and more.

I am filing a formal safety complaint regarding a catastrophic Electronic Power Assisted Steering (EPAS) failure and severe electrical short circuit on my 2015 Ford Mustang. This vehicle is exhibiting hard fault code B137F:09 (Steering Wheel Left Switch Pack: Component Failure / Short to Ground or Open). This electronic failure creates an immediate, severe safety hazard that goes entirely outside the scope of normal vehicle wear and tear. While driving, the vehicle's steering wheel pulls and jerks violently on its own, actively fighting my manual steering inputs. Furthermore, this internal electronic short creates a massive parasitic electrical draw that prevents the vehicle's computer modules from entering sleep mode when turned off. This has resulted in the primary vehicle battery draining completely flat and causing the vehicle to completely shut down, stall, or lose total electrical power altogether. A vehicle that forcefully steers itself away from the intended path of travel and suffers total electrical shutdown is a clear, life-threatening safety risk to myself and others on the road. Ford Motor Company is fully aware of this dangerous defect. They previously issued Safety Recall 15S18 for a very specific, limited batch of early-production 2015 Mustangs experiencing identical EPAS failures, replacing their steering gears for free. For the remaining 2015 Mustang owners experiencing the exact same hazardous failure, Ford quietly shifted the issue over to a Technical Service Bulletin (TSB / SSM 51312). By limiting the scope of Recall 15S18 purely due to the financial cost of a universal recall, Ford has left an entire population of 2015 Mustang owners in direct harm's way. Some owners received a free lifetime safety fix, while the rest of us are being forced to pay thousands of dollars out-of-pocket to correct an identical factory electronic defect. I request that the NHTSA immediately open an investigation into Ford's handling of the 2015 Mustang EPAS and more.

The driver-side seat belt retractor on my 2015 Ford Mustang GT Premium has failed. The seat belt retracts back into the retractor housing when released; however, it does not lock when pulled sharply and continues to extend freely. I confirmed this by repeatedly performing a rapid pull test while seated in the vehicle. The failure affects the seat belt restraint system because the belt does not appear to engage its locking mechanism. This could increase the risk of injury in a crash because the seat belt may not properly restrain the occupant during sudden deceleration or impact. The issue was inspected and confirmed by a Ford dealership service department. The dealer diagnosed the driver-side seat belt retractor as failed and recommended replacement of the retractor assembly. The dealer provided an estimated repair cost of approximately $550 plus tax. The vehicle was inspected by Smith Ford in Conway, Arkansas. The vehicle was also inspected for a separate seat belt recall, and the recall inspection passed. The dealer advised that the failed retractor was not related to the recall and would require customer-paid repair. ENGINE

“Charge system service now” came on the dash, battery lost power after about 10 seconds, screen went black, dash went black, car locked up, opened the hood and the engine was on fire, called 911 who came quickly and put the fire out without the entire car burning, insurance later total losses the car. I don’t know if it was the battery or the engine responsible for the failure, because the flames were on the engine and not the battery, but the incident was triggered by a battery warning. I was on the interstate driving 60mph when the car shut off and everything including my headlights and even hazard lights went black. Not only could I have been struck by a car on the interstate, I could have died in a car fire. Because the car locked up, I couldn’t open the trunk or use any electronics. The door could have just as easily locked and trapped us inside a burning car. The car is on a salvage yard, no attempt to replicate has been made. Progressive insurance, and Columbia fire inspected it and found that it was undetermined. “Charge service system now” was the only warning before the infotainment screen shut down to reduce battery usage. Every light on the dash flashed right before everything on the dash went black and power was lost to the entire vehicle.

With only 32,885 miles, engine light came on due to (P0456) EVAP small leak. Went to an auto shop and Ford dealer for diagnostic scan. Both technicians found that there is a bit of smoke coming out from the top of the fuel tank due to a small stress crack at the port near the canister vent hose as well as a small crack below fuel pressure sensor tube. New fuel tank replacement was recommended as the crack could not be repaired. Cost to fix is $1,400 parts and $1,200 labor. This small crack nears the fuel tank seems to be a major concern for owners as it could increase the risk of vehicle fire and should be a recall by Ford for immediate remedy.
