NHTSA investigations into the 2021 Ford Explorer

An investigation is the step before a recall: the agency has seen enough complaints to look into a pattern itself. An open one means it thinks something may be wrong and nothing has been fixed yet.

Canada — Transport Canada

Canada runs its own recall programme, and its campaigns do not always mirror the American ones.

Recall 2026339 26,900 units in Canada

Seats And Restraints

Issue: On certain vehicles the recall repairs completed by a Ford dealer during recall 25S67 (Transport Canada recall no. 2025-328) may not be effective. As a result, a second repair is required. On certain vehicles, the trim for the seat fold switch on the second-row outboard seats could become stuck. If this happens, the seat(s) could fold and/or move unexpectedly. Note: This recall replaces Transport Canada recall no. 2025-328. Vehicles that were repaired under that recall also require this repair. Safety Risk: A seat that folds and/or moves could increase the risk of injury in a crash. Corrective Action: Ford will notify owners by mail and advise you to take your vehicle to a dealership to replace the seat fold switch trim and housing for one with an improved design.

Recall 2026311 33,012 units in Canada

Powertrain

Issue: On certain vehicles, a problem in the transmission could cause the parking pawl to become damaged. As a result, when the transmission is shifted to Park, the vehicle could move if the parking brake is not applied. Safety Risk: A parked vehicle that rolls away could create the risk of an injury or a crash. Corrective Actions: To reduce the safety risk, Ford recommends that you should always use the parking brake when you park until the recall repairs are completed. Ford will notify owners by mail and advise you to take your vehicle to a dealership to update the powertrain control module (PCM) software. The dealer will also inspect and, if necessary, replace any damaged transmission components.

Recall 2026124 44 units in Canada

Seats And Restraints

Issue: On certain vehicles, the second-row outboard seat belt anchor bolt(s) may be loose or missing. As a result, the seat belt(s) could detach in a crash. Safety Risk: A seat belt that detaches could increase the risk of injury in a crash. Corrective Actions: Ford will notify owners by mail and advise you to take your vehicle to a dealership to inspect and, if necessary, install, tighten or replace seat belt anchor bolt(s).

Recall 2026100 99,375 units in Canada

Visual System

Issue: On certain vehicles, a software problem could cause the rearview camera image not to display properly when the transmission is shifted into reverse. Canadian regulations require the rearview image to display when the vehicle is reversing. Safety Risk: A rearview camera image that doesn't display correctly could reduce a driver's ability to see behind the vehicle while backing up. This could increase the risk of a crash. Corrective Actions: Ford will notify owners by mail. The corrective actions for this recall are under development.

Recall 2026098 54,078 units in Canada

Visual System

Issue: On certain vehicles, the windshield wiper motor may not have been manufactured properly. As a result, the motor could fail and cause the windshield wipers not to work. Safety Risk: Windshield wipers that don't work could reduce visibility and increase the risk of a crash. Corrective Actions: Ford will notify owners by mail and advise you to take your vehicle to a dealership to inspect and, if necessary, replace the windshield wiper motor.

Recall 2026005 138,783 units in Canada

Electrical

Issue: On certain vehicles, the engine block heater could leak coolant. If this happens, the block heater could short circuit when it's plugged in. Note: This recall replaces Transport Canada recall no. 2025-554. Vehicles that were repaired under that recall don't require this repair. Safety Risk: A short circuit can create the risk of a fire. Corrective Actions: To reduce the safety risk, Ford advises you not to use the engine block heater until the recall repairs have been completed. Ford will notify owners by mail and advise you to take your vehicle to a dealership to replace the block heater. Alternatively, owners can choose to have the dealership permanently disable the engine block heater.

Recall 2025554 128,886 units in Canada

Electrical

This recall was replaced by Transport Canada recall no. 2026-005. Issue: On certain vehicles, the engine block heater could leak coolant. If this happens, the block heater could short circuit when it's plugged in.

Recall 2025366 107,534 units in Canada

Fuel System

Issue: On certain vehicles, the low-pressure fuel pump could fail. If this happens, the engine may run rough or may not start and the check engine light may turn on. This could also result in a sudden loss of engine power while driving. Safety Risk: A sudden loss of engine power could increase the risk of a crash. Corrective Actions: Ford will notify owners by mail and advise you to take your vehicle to a dealership to update the powertrain control module software. For Super Duty models, the dealer will replace the fuel pump.

I previously reported my rear view camera not working despite ford having many recalls for that problem. Now my shade for my sunroof when opened drops into my car & blocks my view from my rear view mirror. So now I have no camera view or physical view of what's behind me. I also have the same peeling metal on my buttons on my driver's door. The same one that was recalled on 2020 model. I cut my arm on it yesterday

ELECTRICAL SYSTEM

I am reporting a persistent and escalating electrical safety defect in a 2021 Ford Explorer. On February 28, 2022 (at approximately 10,542 miles), I formally reported a strong burning metal smell and the engine would be loud and idle high, there was abnormal noises and unusual electrical changes to the dealership (West Herr Ford). The dealer service advisor documented no problem found and told me not to worry about it. Over time, this unresolved electrical short and metal smell has resulted in a total system failure. In February 2026, the vehicle suddenly suffered a complete ‘black screen’ (APIM/infortainment failure) and a no-start condition. Most dangerously, the battery suddenly drains within days, which can lead to total loss of vehicle power while in motion, disabling power steering, braking assists and you can’t even put the car in neutral when this happens. I am gravely concerned that this unaddressed metal burning smoking/ electrical short issue poses a significant potential fire risk to myself, my passengers and other drivers. The same dealership has now had this car in there possession for 32 days and admits that what has happened is what happens while they have it and that they cannot find the root cause. Yet the previous service advisor from 2022, and the current service advisors ( gave the car back on 2/21/2026 and 3/6/2026 saying their was no problem) allowed the vehicle to remain on the public roads despite documented fire hazard symptoms.

UNKNOWN OR OTHER

My vehicle is experiencing exhaust fume issue where the exhaust fumes are entering into the cabin for Explorer and Aviator models (Any model with the 3.0L Ecoobost). Upon inspection by 3rd party mechanic, it can be determined that the problematic catalytic converters that are not the one piece cast was installed on my vehicle from the factory. Speaking to other ST and Interceptor owners, the problem seems widespread to every 2021 model that Ford installed the multipiece welded catalytic converters. Apparently the cast part fairs better and is not subject to cracks. The recall originally covered the left side catalytic converter but the flawed design also affects the right side catalytic converter where many owners are experiencing this issue. This has affected all 3.0L Ecoboost engine vehicles. [XXX] I believe the recall should have extended to cover all 2020 and 2021 model year vehicles. There are forum posts where owners outside the recall have experienced the issue in addition to owners having to repeatedly replace the catalytic converters post warrant as Ford continued to reinstall bad catalytic converters as part of the repair process. I have driven this vehicle for several thousand miles with the issue and did not realize exhaust was leaking until notices of headaches and health issues as CO is odorless, causing CO2 levels to reach 3000+ inside the cabin at stop lights and blows in through the vents if A/C is on. I believe NHTSA should investigate this issue further and require Ford to replace both catalytic converters as this issue poses a serious health hazard, especially given the location of the exhaust leak and the commonplace occurrence. I am happy to forward to forums and provide evidence of this occurring. Ford considers lifetime of the vehicle to be ~150K miles and these exhausts do not last close to that, resulting in a massive expense and danger to US consumers. There was no accident, but very close a few times.when I am stopped at a stop sign or light when I press down on the gas pedal the car hesitates I have to pump the pedal to get the car moving or it would stall in traffic. No check engine light is or has been on. I’ve been to 2 different ford dealerships and after there inspection they claim nothing wrong with my car. It’s already cost me a lot if money. When driving I can feel the car surge likes it’s not getting fuel sufficiently real bad at traveling at or below 50 mph. I’m afraid it will stall and get us t- boned but the mechanics say nothing wrong I disagree there is something wrong. I know another person who had the same problem and he got rid of the car, I can’t afford to do that, please do something to get a recall or something before we’re seriously injured or worse.this started about a year ago and getting worse.

ENGINE

The intermediate shaft has gone bad on my 5 year old car. Noticed it on 12/11/25 after owning for 1 year. I’m slowing losing oil which then can cause my engine to go out. I have read many things stating other peoples frustration with this $3.59 part going bad in the 2021 ford explorers but to get it fixed you have to drop the entire engine resulting in it costing $1,800. I think ford should be responsible for this and aware to make a better part.

Recently my car has had a series lights come on including 4wd fault, pre-colliosion assist not available, service advance trac, hill start assist warning, and something with the brakes when in reverse. I took it to the dealer and they said it's something electrical that will need parts, and further investigation with removing the seats. That will be another day. Today, while driving with my son in the back, I was taking a u-turn on a busy street. The car acted like it switched into neutral (but didn't), and wouldn't accelerate, leaving me inching into traffic at 1 mph with cars going 40mph coming at me. Luckily I was not hit, but easily could have been. I drive almost 60 miles a day for work and this is a safety hazard not just for me, but my family too.

Luckily I was not hit, but easily could have been. I drive almost 60 miles a day for work and this is a safety hazard not just for me, but my family too.

FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING

Recently my car has had a series lights come on including 4wd fault, pre-colliosion assist not available, service advance trac, hill start assist warning, and something with the brakes when in reverse. I took it to the dealer and they said it's something electrical that will need parts, and further investigation with removing the seats. That will be another day. FUEL/PROPULSION SYSTEM

I parked my car and when I returned to it the car would not start. Diagnosed as a faulty fuel pump. Had this problem occurred while I was driving it could have caused serious injury or death. There is an active recall on fuel pumps for the 2021 Ford Explorer that does not apply to my VIN number so Ford will do nothing to remedy the situation and parts are impossible to find as dealers scramble to fix the vehicles included in the recall.

I am reporting a safety concern involving a delayed transmission gear engagement and delayed rearview camera activation. Intermittently, when shifting the vehicle from Park into Reverse or Drive, there is a noticeable delay before the transmission actually engages the selected gear. During this delay, the rearview camera also fails to activate immediately. Because there is no immediate indication that the requested gear has engaged, I have experienced situations where I believed the vehicle was in Reverse or Drive, but it remained in the previous state for several seconds. In some instances, the transmission engages unexpectedly after the delay, causing the vehicle to move in a direction different from what I anticipated. For example, after selecting Reverse, the vehicle may hesitate before moving, or after changing between Reverse and Drive, the delayed engagement can result in the vehicle moving in the previously engaged direction before finally shifting into the selected gear. This creates a significant safety hazard, particularly when parking, backing out of driveways, or maneuvering in areas with pedestrians or other vehicles. The delayed rearview camera activation further increases the risk because it prevents the driver from immediately seeing behind the vehicle while attempting to reverse. The issue occurs intermittently and without warning, making it difficult to predict. I am concerned that this condition could lead to a crash, property damage, or injury if the vehicle moves unexpectedly due to the delayed gear engagement. I request that NHTSA investigate this issue to determine whether it is related to a transmission, electronic shift control, or software defect affecting the safe operation of the vehicle.

The vehicle has been failing shifting. There is something from with failure to shift. The vehicle won’t accelerate, and slows down in the middle of traffic and the truck has no other problem. I’ve only have it for 3 years with no problems. But this is a big safety issue while driving

FUEL/PROPULSION SYSTEM

I was traveling on an 8 hour drive after just getting a new battery and fresh oil change, I also had a full diagnostic ran the week before travel and no issues were found in my vehicle. When we arrived at our destination we stopped 1 final time and attempted to start our vehicle to go to dinner and the car would not turn over. There were not any warning lamps or messages that appeared. We used roadside assistance to tow it to the [XXX] dealership and they ran another full diagnostic that showed "replace fuel pump and fuel pump driver". When I called my local dealer back home, Evansville Ford ([XXX]) they pulled the previous week diagnostic and confirmed nothing showed up regarding the fuel pump at that time. 1 week later is when we had it in [XXX], and the fuel pump failed.
